Since April 2016, NHS and local authorities across Scotland have been working towards the integrated provision of health and social care within their territorial areas and over 30 partnerships have been set up already for this purpose.

The idea that there is “no better ward than home” is a great concept that comes with a number of information sharing challenges. The partners must agree on the information governance structures and processes, but also to ensure the information is available when needed at any point in the integrated health & social care pathway,  whilst protecting the privacy rights of those involved and ensuring adequate organisational and technical security measures are in place.

This process involves the blend of services, processes, technologies and information currently spread across the complexity of Health Boards and Local Authorities, requiring re-design and dialogue between the partners to reach beneficial outcomes.

This webcast presents the philosophy behind the Scottish Information Sharing Tool-Kit and how this initiative can help partner organisations all the way through this process,  by facilitating their decision making with respect to the sharing and processing of information, lessening information risks whilst stimulating the confidence of users and,  enabling the expected efficiency from those integrated services.
