The “Connect Me” programme for self-care at home.
If you test positive for Covid-19 and you are being discharged from hospital, or you are pregnant, your clinician/midwife may advise you that you are eligible to take part in remote monitoring at home.
This programme, also known as “Connect Me”, allows you to self-manage in your own home with advice on the use of monitoring equipment and guidance on when/how to seek further medical attention.
If you consent to participating in remote monitoring, upon registration you will be asked to provide personal information such as your name, address and date of birth. This will be used to communicate with you throughout the period of remote monitoring at home.
